Have you recently received a MRI, X-Ray, CT Scan, Ultrasound, or Mammogram (from any state) or Emergency Service (from New Jersey only) AND have commercial health insurance?
If so, we will ask you for the following information:
* Name and Location of Facility
* Type of Medical Service Received
* Contact Info
Send in your EOB
If eligible, we’ll ask you to email us a photo of your Explanation of Benefits (EOBs).
The EOB should contain:
* The amount covered by your insurance
Get Paid

What is an EOB (Explanation of Benefits)?
An Explanation of Benefits, which is often referred to as an EOB, is a statement sent by health insurance companies to covered individuals that explains how the individual’s benefits were applied to a particular claim and what treatments and services were paid for by the insurance company on the individual’s behalf. An EOB typically includes the name of the insured patient, a description of the services rendered, the provider who billed for the services, the amount of fees incurred, and the amount covered by the insurance plan.
Will my personal information be safe?
We take our users’ privacy very seriously. As such, your private information will be used solely for the purposes of this study. We will NEVER share your personal information with third parties.
How quickly do I get paid?
We’ll send payment within 2 business days once we receive all qualifying information. This includes receiving all EOBs related to a particular imaging service, the provider who billed for the service, legible cost information, and sufficient data to link cost to a specific imaging procedure. Denied claims are ineligible for the study.
Please note this excludes all government and state funded health plans in addition to any Medicare Advantage or Medicare Supplement Insurance sold by private companies.
Who is this study for?
This study is for anyone who is commercially insured and has received an imaging service (including CT Scans, MRIs, X-Rays, Ultrasounds, and Mammograms) from any state or emergency service (from New Jersey only) within the last 24 months at a qualifying hospital or hospital-based facility. Please note this excludes all TRICARE, Medicare Advantage, and Medicare Supplement plans.
