WHAT INFORMATION ABOUT YOU DO WE COLLECT AND HOW IS IT USED?

We may ask for certain personal information from you for the purpose of healthcare cost research. We collect Personal Information in the following ways:
Initial Contact Information: If you indicate an interest in participating in a particular research study, you are asked to contact us by clicking on the “Contact Us” link and submitting your name, phone number and e-mail address, which we retain on file, so that we may contact you to learn whether you satisfy the criteria for that particular study. We may retain a record of users who have contacted us in order to respond properly to questions or concerns and for purposes of future communications unless you do not qualify for a particular study and indicate to us that you do not want to receive any future communications.
Medical Information: If you satisfy the criteria for that particular study, we request medical information sent via email, fax, or mail. We respect the privacy of our participants and will never ask for their date of birth, social security #, health insurance policy #, claim # or any other personal information. We prefer the patient to mask or black out such personal data with a felt tip or other marker before sharing cost data with us. The only personal information we will ever need is your name and email or mailing address to send you payment for your participation. The health cost information that you provide us is summarized for computational purposes only.
Your information is secure and will never be shared or sold with third parties. Healthcare Cost Research Group does not publish, nor make public any of your information, which of course includes information that will compromise the identity nor the security of our research participants. Our employees, agents and contractors who have access to personally identifiable information are required to protect the information in a manner that is consistent with this Privacy Statement.
Non-Personally Identifiable Information We Collect.
Cookies/ Web server logs: Similar to other commercial Web sites, our Web Site utilizes standard technology called “cookies” and Web server logs to collect information about how our Web Site is used.
